This form is to be filled out any time there is
(1) an injury in connection with the activities of Revelstoke Youth Soccer (any injury serious enough to require medical attention)
(2) an incident involving a player, team official, team, game official or spectator that may require disciplinary action
Reported incidents will be reviewed by a Discipline Committee, which may consist of Board members, staff, or other designated individuals as appropriate to the nature of the incident.
Incident Report Forms should be submitted via this form or a detailed email (contat@revsoccer.com) as soon as possible and no later than 24 hours following the incident. Upon receipt of the report, the Discipline Committee will acknowledge the submission and begin its review. A response regarding next steps or the outcome of the review can generally be expected within three (3) business days, although more complex matters may require additional time.
